If you have received an invitation to join this Honor Society, or if you believe you are eligible to join Phi Theta Kappa, please download an official application for membership.  After you complete this application you must return it to your Campus Phi Theta Kappa Advisor.  Arrangements to pay the $70 induction fee should be made with your Campus Phi Theta Kappa Advisor.
