Faculty Referrals

Counseling Referral Form

The Counseling Referral Form is a mechanism by which any faculty person may refer a student to a DMACC Counselor. Reasons for referral may include:

• Unacceptable academic performance
• Unexplained absences
• Family issues
• Mental health concerns

• Re-evaluation of goals
• Career counseling
• Adjustment issues

Whenever possible, faculty members are expected to address individual concerns with students in private prior to making a Counseling Referral. Notation of this discussion, feedback and attempted strategies are documented by the faculty person on the referral form. (Please note: This is not the mechanism used to report “failure to attend” to the Registrar’s Office.)

Referrals may be submitted in paper form or electronically directly to an individual Counselor or to Faye Johnson, Director of Student Development. In some cases, the Director of Student Development may re-distribute referrals for purposes of balancing workload. Counseling Referral Forms will be made available to faculty at the beginning of each semester. Additional copies are available in faculty workrooms, from any Counselor, and electronically at http://www.dmacc.edu/Counseling/.

A Counselor receiving a Counseling Referral Form will acknowledge receipt by e-mailing the referral faculty person, indicating only the date a referral has been received. Counselors (professionally bound by the American Counseling Association Code of Ethics, Section A.6.b. Confidentiality and Advocacy) will treat all counseling interactions with the student in a confidential manner. When further communication with the referring party regarding counseling intervention is deemed necessary, Counselors must obtain client consent.

A list of Counselors, locations, and program assignments are available to all Deans, Directors, and faculty for use in the referral process. Use of the Counseling Referral Form does not preclude in-person referrals or those of an urgent nature.
